How to complete a Provider Match form
Step 1 — Access the Provider Match form
Go to the Provider Match form. (Tip: save the link as a bookmark for easy access.)
Step 2 — Complete Your Details
This section covers how we can contact you to discuss your enquiry if we need more information.
Step 3 — Complete Service Details
This section covers what service you're looking for and who will be receiving the care. The more detail you include under Health Diagnosis, the more likely we are to find a great match for you.
Step 4 — Complete Additional Requirements
Include any preferences for your provider that we can help enquire about — for example, language or gender preferences, or a company that's LGBTQI+ friendly. Let us know about any specialisations you'd prefer from your provider too, so we can find the right match.
If there are complex needs involved in this enquiry, tick the checkbox at the bottom of this section and one of our team will call you to discuss the details.
Step 5 — Complete your request
Once you've filled in the form, select Submit to send it to our team to start sourcing providers for you.
You'll receive updates along the way, and should expect your shortlist within 48 hours, sent to the email address you entered above.
