How to claim your provider profile
- Go to https://www.kismet.healthcare/providers and search for your provider name.
If you cannot find your profile, please complete this form and our team will be able to help you.
- Select Claim this provider

For security reasons, you will need access to the email registered against the listing.
If you do not have access, please complete this form and our team will be able to help you. - Select I Do and an email will be sent to the listed address.
- Open your email Inbox listed and find the code sent from us.
- Enter the code and click Complete Claim
